Client Service Agreement

This contract must be signed by the primary client prior to every session — no exceptions. It is valid for one session only. A new agreement is required at each subsequent booking.

⚖ Automatic Agreement — Read This First

By requesting, inquiring about, depositing for, or otherwise engaging with Jezebel's Maid Adult Entertainment in any capacity, the Client automatically agrees to all terms, policies, and boundaries contained in this agreement. This applies regardless of whether a physical signature has yet been obtained. Ignorance of these terms is explicitly not a valid defense. The physical signature below confirms receipt, understanding, and continued agreement.

Section 1

Nature of Service

This agreement governs a legitimate adult entertainment and light housekeeping service provided by Jezebel's Maid Adult Entertainment ("Provider") to the client identified above ("Client"). This service operates within the bounds of applicable Indiana and Illinois state law.

No sexual services are offered, implied, or available under any circumstances whatsoever. Any client who books with the belief or intent that sexual services are included is in breach of this agreement from the moment of first contact and forfeits all payments made.

Section 2

Automatic Agreement & Binding Terms

By engaging with Jezebel's Maid Adult Entertainment in any capacity — including but not limited to: making an inquiry, submitting a deposit, scheduling a session, or arriving at a session location — the Client agrees to be fully bound by all terms of this agreement.

Claimed ignorance of any term or policy is explicitly not a valid defense. This agreement constitutes the entire understanding between parties. No verbal agreements, implied terms, or modifications made outside of this written document are enforceable.

Section 3

Age Verification & Screening

All individuals present during any session must comply with the following without exception:

Be a minimum of 21 years of age
Present valid, government-issued photo ID at the time of booking confirmation and again at the door on session day
Guests arriving after session start must also present valid ID before being admitted to the space
Any individual unwilling or unable to produce ID will not be permitted in the space — Provider may proceed with verified guests only or terminate at her discretion
Discovery of any underage individual on premises results in immediate session termination, full forfeiture of all payments, and may be reported to law enforcement
Client ID and session details are retained securely for a minimum of 90 days following each session

Section 4

Booking, Payment & Rescheduling

A non-refundable deposit of $50 is required to confirm all bookings. This deposit is applied toward the session total.
Remaining balance is due in full at the start of the session, before any services begin. Provider reserves the right to refuse service if balance is not tendered upfront.
Rescheduling with 48 or more hours notice incurs no penalty — deposit transfers to the new date.
Cancellations with less than 24 hours notice result in forfeiture of the deposit.
No-shows are billed a $200 minimum fee. A card on file may be required at booking.
Extended travel (up to 60 min outside Crown Point or Chicago service area) incurs a flat $55 travel fee due at booking.
Long-distance or unusual travel must be quoted and agreed to in writing prior to booking confirmation.
Cleaning supplies, if requested from Provider, incur a flat $20 fee.
Costume or special attire requests are subject to availability and separately quoted prior to booking.

Section 5

Guest Policy & Occupancy

Standard sessions accommodate a maximum of 2 guests at the standard cleaning rate
3 or more guests constitutes a party booking and must be billed at the party hosting rate — no exceptions
Additional guests arriving on the day of service not disclosed at booking will trigger the party rate difference, due immediately, or the session terminates
All guests — including latecomers — must present valid 21+ ID before entering the session space
The primary Client is fully responsible for the conduct of all their guests. Guests are equally bound by all terms of this agreement

Section 6

Scope of Cleaning Services

Where a cleaning session has been booked, services are strictly light housekeeping only:

Included: Washing dishes, wiping down kitchen surfaces, vacuuming floors
Not included: Deep cleaning, scrubbing, heavy lifting or moving furniture, outdoor work, bathroom scrubbing, dusting, laundry beyond light folding (if mutually agreed), hazardous materials, or any task outside the agreed scope
Client is responsible for ensuring the space is safe to work in. Provider reserves the right to decline tasks deemed physically hazardous
The Provider is not responsible for pre-existing damage to Client property
Client must have supplies accessible and space reasonably prepared before Provider arrives

Section 7

Session Atmosphere & Client Conduct

Client may set the mood and atmosphere of their session — music, lighting, and general ambiance are at Client's discretion within the following standards:

All clients and guests must remain fully clothed at all times for the duration of the session
No alcohol may be offered to or consumed by the Provider during any session
Client may request a Social session (conversation welcome) or a Quiet session (minimal interaction) — this preference must be noted at booking
Respectful, professional communication is required at all times regardless of session type
A private changing space must be available for Provider upon arrival — free of cameras, recording devices, and other individuals
No following Provider to or loitering near the changing space
No attempts to direct, pressure, or coerce Provider into tasks or behaviors outside this agreement

Section 8

Absolute Boundaries — Zero Tolerance Policy

The following violations result in immediate session termination, forfeiture of all payments, and potential legal action. There are no warnings, no second chances, and no exceptions:

No touching — physical contact of any kind with the Provider is strictly prohibited, including contact framed as incidental or accidental
No solicitation — requesting, suggesting, hinting at, or negotiating for sexual services in any form
No recording — photography, video, or audio recording of the Provider without explicit prior written consent. Recording devices are never permitted in the changing area
No minors — presence of any individual under 21 on premises at any time during the session
No intoxication pressure — offering, pressuring, or attempting to cause the Provider to consume alcohol or any substance
No verbal abuse — demeaning, threatening, or harassing language directed at the Provider
No obstruction — blocking, detaining, or impeding the Provider's ability to leave at any time for any reason

Violation Consequence: Session terminates immediately. All payments are fully forfeited. A minimum violation fee of $500 is assessed and immediately due. Physical contact or assault will result in law enforcement contact. Provider reserves the right to pursue all available civil and criminal legal remedies without limitation.

Section 9

Provider's Right to Refuse or Terminate

Provider reserves the absolute, unconditional right to:

Refuse any booking, at any time, without explanation or obligation to refund any deposit paid
Terminate any session immediately if she feels unsafe, disrespected, or if any term of this agreement is violated or appears at risk of violation
Leave the premises immediately upon session termination — Client agrees not to block, delay, or interfere with Provider's exit under any circumstances
No refunds are issued for sessions terminated due to Client or guest violations

Section 10

Workspace Safety

While the Provider is present to perform entertainment and light cleaning services, Client acknowledges a duty to provide a reasonably safe working environment. Specifically:

The session space must be free of obvious physical hazards that could cause injury to the Provider
Client must disclose any known hazards (broken fixtures, unstable surfaces, animals that may pose a risk) prior to the session
Provider may decline to enter or may terminate a session where the space is deemed unsafe — no refund will be issued in this circumstance if Client failed to disclose
Provider does not carry cleaning insurance — Client assumes liability for any pre-existing property conditions

Section 11

Privacy, Confidentiality & Anonymity

Client agrees never to share, post, publish, or distribute the Provider's personal information, contact details, likeness, location, or any images or recordings from any session
Provider agrees to maintain strict confidentiality regarding Client identity, session details, and location — this information will never be shared with third parties except where required by law
No photography or video of the Provider is permitted under any circumstances without explicit prior written consent — this is a hard, non-negotiable boundary
Violation of this clause constitutes grounds for immediate legal action including pursuit of damages
Both parties acknowledge that mutual anonymity and discretion are foundational to this service and agree to uphold them unconditionally

Section 12

Safety Protocol

For the protection of the Provider, the following safety measures are in place at all times. Client acknowledges their existence and agrees not to interfere with them:

Provider operates with a designated trusted contact who is informed of all session locations, scheduled start times, and expected check-in times
Provider performs a safety check-in at the start and end of every session
If a check-in is missed, the trusted contact is authorized to contact law enforcement and provide the Client's session address and ID information collected at booking
Client ID and session records are retained securely for a minimum of 90 days post-session

Section 13

Liability & Governing Law

Jezebel's Maid Adult Entertainment is not liable for property damage that occurs in the normal course of light cleaning services
Provider is not liable for pre-existing damage to Client property
Client assumes full legal responsibility for the conduct of all guests present during the session
This agreement is governed by the laws of the State of Indiana
Any disputes arising from this agreement shall be resolved in the courts of Lake County, Indiana
This agreement constitutes the complete and exclusive understanding between the parties — no verbal modification is enforceable

Acknowledgment & Submission

By checking the agreement box and submitting above, Client confirms they have read, understood, and fully agree to every term of this agreement. Client confirms all guests present are 21+ with valid ID and are equally bound by these terms. This submission is legally binding. Provider will countersign digitally before the session begins and you will receive a confirmation email.

For In-Person Sessions

A physical copy of this agreement will also be signed at the door before any session begins. Provider retains the original. Client receives a copy.
